By Telephone
Please call 480-629-8882 Monday through Friday 8:00am to 5:00pm. Our friendly office staff will assist you by arranging your saliva hormone testing and paperwork needed to get started. You may choose to stop by the office to pick up your hormone testing kit or have us send it to you priority mail. Once we receive your results from the lab we will contact you to schedule your initial appointment. This is typically 3-7 business days after the lab receives your samples.

Online Registration
Simply complete our short online registration form to create your account. Once completed you will have access to download paperwork to get started. You may fax or email your paperwork back to us after completion. You may choose to stop by the office to pick up your hormone testing kit or have us send it to you priority mail. Once we receive your results from the lab we will contact you to schedule your initial appointment. This is typically 3-7 business days after the lab receives your samples.
